DAISY Award for Extraordinary Nurses

How to Nominate an Extraordinary Nurse

Patients, their families, and caregivers may nominate a deserving nurse.

There are three ways to nominate a nurse for the DAISY Award:

  • Fill out this online nomination form and click submit
  • Download and complete this nomination form and place the form in the box in the front lobby of the hospital OR hand your nomination to a hospital director
  • Complete this nomination form and mail to: Regional Health, Rapid City Hospital, Nursing Administration, 353 Fairmont Blvd., Rapid City, SD 57701

Award criteria include:

  • Must be a direct-care nurse at the bedside
  • Must be nominated by patient, family, or caregiver
  • Must be:
    • Compassionate
    • Kind & Sincere
    • Trustworthy
    • Passionate
    • Caring

About The DAISY Foundation

The DAISY Foundation was established in 1999 by the family of J. Patrick Barnes who died of complications of the auto-immune disease Idiopathic Thrombocytopenia Purpura (ITP) at the age of 33. (DAISY is an acronym for diseases attacking the immune system.) During Patrick's eight-week hospitalization, his family was awestruck by the care and compassion his nurses provided not only to Patrick but to everyone in his family. So one of the goals they set in creating a foundation in Patrick's memory was to recognize extraordinary nurses everywhere who make an enormous difference in the lives of so many people by the super-human work they do every day.

What Is The DAISY Award?

daisy-award.jpgPatrick's family brainstormed what to do in his memory. They vividly recalled the skillful and compassionate care Patrick received from his nurses during his eight-week hospitalization. They wanted to say "thank you" to nurses everywhere by establishing a recognition program - The DAISY Award for Extraordinary Nurses - to honor the super-human work nurses do every day at the bedside.

The DAISY Award is a nationwide program presented in collaboration with The American Organization of Nurse Executives (AONE) that rewards and celebrates the extraordinary clinical skill and compassionate care given by nurses every day. Rapid City Regional Hospital is proud to be a DAISY Award hospital partner, recognizing one of our nurses with this special honor every month. Rapid City Regional Hospital has committed to the DAISY Award and will be honoring one nurse per month with a certificate, pin, tote bag, and a Healer's Touch stone carving.
